Texas State Immunization Requirements
| Diphtheria/Tetanus/Pertussis Diphtheria/Tetanus |
Effective August 2004: Upon entry into Kendergarten, students must have five (5) doses of vaccine. (5th dose not required if 4th dose given on or after fourth birthday Pre-K and under must meet age appropriate requirements. Students seven (7) years and older a minimum of three (3) doses. Diphtheria/Tetanus booster due every ten (10) years. |
| Polio | Effective August 2004: Upon entry into Kindergarten, students must have four (4) doses of vaccine with one on or after fourth birthday. Or, if third dose was given on or after the fourth birthday only three (3) doses are reauired. Pre-K and under must meet age appropriate requirements. |
| Rubeola (Red Measles) | Two (2) doses of vaccine are required for all students with the first dose being after the first birthday, and the second dose must be at least 30 days after the first. |
| Rubella (German Measles) | One (1) shot of vaccine required for all students on or after the first birthday. |
| Mumps | One (1) shot of vaccine required for all students on or after the first birthday. |
| HIB (Haemophilus Influenza) | One (1) shot is required for all students 15 months through four (4) years of age unless a schedule for a primary series and a booster was completed prior to or at 15 months of age. |
| Hepatitis B | Effective August 2004: Early childhood through 5th grade and 7th grade through 10th grade must have three (3) doses. |

Pre-K and under must meet age appropriate requirements. Two doses 30 days aprt for students 13 years old or older without a disease history. |
